“Apple has today announced a replacement program for some European iPhone USB adapters. The company says that it has identified that some USB adapters bundled with iPhones (as well as sold separately) may overheat and ‘pose a safety risk,'” Benjamin Mayo reports for 9to5Mac. “The adapter shipped with European models of the iPhone 3GS, iPhone 4 and iPhone 4s from October 2009 to September 2012.”
“Affected adapters feature the Model ‘A1300′ designation, found on the bottom of the unit,” Mayo reports. “Apple will replace the power adapters for free, at its dedicated replacement portal website.”

Only Apple would replace 4-6 year old adapters for free. Most makers consider it a non-issue once things are out of warrantee.
Apple replaced my iPod nano first gen unit due to a possible battery issue….. 5 years after I bought it. But there was one problem, like others mine still worked so I had kept it. Apple ran out of replacement batteries…… So they GAVE ME an iPod nano 6th gen to replace it…… NO CHARGE.

Not sure if it “only” Apple, but this replace program is definitely right thing, as adapters can be dangerous, if they overheat under certain conditions.
The very fact that Apple has decided to replace adapters that are not produced for more than two years already, so far did not cause any noticeable issues, and could only be a problem in mostly theoretical circumstances, is showcase of class that Apple offers.
If it is dangerous then their obligation under EU law is to withdraw them.
